Westland Convalescent & Rehab Center's Subacute Care Program provides residents with cost effective transitional care between hospital and home.
Our Specialty Care Coordinator, with a background in nursing is highly experienced in both post acute and long-term care. She will come to you, at the hospital or home, to answer questions to streamline the transition to Westland Convalescent & Rehab Center.

Judith G. Caroselli, RN, Chief Operating Officer, A former critical care nurse who came to WCC in 1972 as the Assistant Director of Nursing. Promoted to Director of Nursing in 1975 and has been the Administrator since 1972. Judy won the "President's Award" an unprecedented two times from the Health Care Association of Michigan, and the "Distinguished Administrators Award" from the American College of Health Care Administrators. She is poised to challenge the staff each day to excel by providing the talent and resources needed to meet the acuity levels of today's short term rehabilitation clients while assuring excellence for those who may require extended services. |

Dr. Keith Tobin DO, is the Medical Director. He and his partners make up what is known as the American Geriatric Consultants. This group utilizes Clinical Nurse Practitioners/Certified Specialist to augment the delivery of services to our residents. |
