The Social Services Department acts as a liaison between the patient, family, staff and community agencies. They provide optimum services to the patient to enhance their psycho-social well being, including the following:

• Counsel patients with psycho-social needs; adjustment to the Center, room transfer, roommate concerns, impending discharge and assist patient/family with plan of care.
• Assist with financial concerns; Medicare, Medicaid, etc.
• Assist with discharge planning:
  • Meet with patient/family regarding their needs in the community.
  • Consult with physician, nurse practitioner, therapy staff and nursing staff.
  • Contact home health care agency of choice.
  • Arrange for all durable medical equipment
