The Social Services Department acts as a liaison between the patient, family, staff and community agencies. They provide optimum services to the patient to enhance their psycho-social well being, including the following:
- Counsel patients with psycho-social needs; adjustment to the Center, room transfer, roommate concerns, impending discharge and assist patient/family with plan of care.
- Assist with financial concerns; Medicare, Medicaid, etc.
- Assist with discharge planning:
- Meet with patient/family regarding their needs in the community.
- Consult with physician, nurse practitioner, therapy staff and nursing staff.
- Contact home health care agency of choice.
- Arrange for all durable medical equipment
Stephanie Myers, BLS, Director, Social Services, graduated with a Bachelor’s Degree from Hillsdale College. She began at Westland Nursing Rehab Center as a volunteer, worked as an Activities Assistant and then became the Social Service Director. Stephanie has many years of experience with geriatric clients and their special needs. |

Carrie O’Connor, MSW, Social Worker, earned her Bachelor’s Degree in Social Work from Mary Grove College and also graduated from Wayne State University with her Master’s Degree. With 25 years of experience, Carrie has worked in foster care and adoption for 15 years, was a substitute teacher for the Redford Union schools and has been in short term and extended care facilities since. |

Jennifer Still, BSW, Social Worker, graduated from Madonna University with a Bachelor’s Degree in Social Work. Due to an eclectic background working in a variety of senior programs, she is knowledgeable in many various issues affecting the senior community today. |
