Every person admitted to Westland Convalescent & Rehab Center must have the recommendation of a licensed physician and the approval of our Admissions Committee. Only those residents that we feel will benefit medically and socially from our care and services will be admitted. Once admission has been approved our Specialty Care Coordinators and Admission staff will work directly with you to arrange the entire move. Our personal involvement ensures a smooth transition with the highest level of professionalism.

FAMILY ORIENTANTION

Regardless of the reason for choosing Westland Convalescent & Rehab Center, the decision is never an easy one. To assist our families with this transition we offer a one hour Family Orientation Program about three weeks after admission. At this time, we discuss:

• WCC Mission Statement
• WCC Fact Sheet - rooms, services, nursing care, physician services, staff, therapies
• Customer Service Connection Sheet, resident rights, concerns for unit managers, staffing and activities
• Advance Directives
• Transition to another unit when Medicare ends
• Questions and answers
